Please bring the completed form with you on the day of your appointment.
PLEASE NOTE: In order serve all of our patients as fairly and efficiently as possible, we ask that you please be respectful of booking times for both testing and doctors appointments.
• Missed testing appointments will result in cancellation of your scheduled appointment with the doctor.
• Cancellation fees will apply to missed appointments or cancellations less than 48 hours before your scheduled visit
